Jones County Healthy Carolinians (JCHC)

Our Healthy Carolinians Partnership, Jones County Healthy Carolinians is a partnership between several organizations and individuals. The mission statement of Jones County Healthy Carolinians is: Working together toward a healthier Jones County through addressing community health and environmental needs.
HEALTH PRIORITIES
In 2006, a Community Health Assessment was completed to determine the health concerns of Jones County. Based on the results of the Assessment, Jones County Healthy Carolinians choose two priority areas, Chronic Disease and Physical Activity. JCHC partners with their network to address initiatives that will improve the health and safety of the community.

PROGRAMS
M.I.R.A.C.L.E. (Managing Insulin Resistance, A Changing Life Experience)
Jones County Healthy Carolinians has developed M.I.R.A.C.L.E., a free diabetes education program. This church-based health literacy program provides prevention and management education workshops, food literacy, blood pressure and weight screening education and one-on-one screening and education sessions for persons with diabetes or at-risk for diabetes. The goal of this project is to increase the capacity within Jones County to prevent, diagnose and medically manage diabetes among African Americans. Because Jones County has such a large faith based minority population, key to the implementation of this project is a population-based strategy focusing on church involvement in each of the communities where many individuals age 18 and older are at risk for type II diabetes.
H.E.A.R.T.S. (Healthy Enthusiastic Active Responsible Teachers and Staff)
H.E.A.R.T.S. is a general wellness program for the teachers and staff of Jones County that focuses on nutrition and physical actvity.
Annual Walk 4 Your Health Walk-A-Thon
Jones County Healthy Carolinians developed the annual Walk 4 Your Health walk-a-thon. Walk 4 Your Health is working to increase physical activity among the adult population in Jones County.
Jones County Employee Wellness Program
Jones County Healthy Carolinians developed the Jones County Employee Wellness Program. This program promotes healthy weight loss for each participant by increasing their daily physical activity and encourages healthy snacking while at work.
Annual Health Summit
The Annual Health Summit was developed by Jones County Healthy Carolinians to provide an opportunity for county residents to voice their community concerns to the staff of the Jones County Health Department and other community organizations.
